I have been a patient at Dr. Orfali’s for over 20 years. He has always been professional and never performs anything that is not needed. I’ve had a couple of crowns and he has done a great job. My adult son was told by a dentist that he had about $5000 worth of work to be done on his mouth. Dr. Orfali reviewed his x-rays and he did not need any work. I have had a couple appointments with Dr. Orfaly at this point, and he and his staff have been joys to work with. They always answer my questions in plenty of detail - but not too much as to confuse me - and are accessible if I need anything. Even further, I had a new crown recently and Dr. Orfaly checked in a couple times after the various steps in the procedure (it was multiple appointments) to ensure everything was good. It is a great personal touch.

I have been a patient of Dr. Fisher and now Dr. Orfaly for over 4 decades. The quality of dental care both preventative and corrective (crowns, root canal, fillings) have stood the test of time. Both Dr. Fisher and Orfaly have terrific chairside manner, answer detailed questions, check with you constantly through difficult procedures and follow-up as needed. The dental assistants and hygienists are professional and enjoy what they do, and also are able to answer detailed questions from everything from their new digital xray system to dental products and aids. Their new front desk coordinator Kerrin, welcomed me by name and had my new appointment scheduled quickly when checking out.

Our family of 5 has been going to this team for 20+ years. Cheryl is a gem and she deserves to be mentioned first. She'll do whatever it takes to help you get the appointment you need (like when your child comes home unexpectedly from college and needs a dental appointment). Dr. Orfaly is my hero as he saved my son's front tooth that was knocked out and dangling from a wayward elbow at the last game of the soccer season. Dr. Fisher's love of Blondie cannot be matched and he's in charge of the music. Feel free to sing along if you are able. Plus he's entertaining in many other ways - ask him to show you his before and after teeth whitening demonstration. What Are Dental Crowns And Dental Bridges?

A dental crown or a dental cap is simply a manufactured tooth placed over a damaged tooth to cover it and restore its shape and size, strength and improve its appearance. The dental crown protects the tooth structure in a way that cannot be done by fillings or other types of restorations. 

Dental bridges are prosthetic appliances that replace missing teeth. While a dental crown is sufficient when the patient has lost a single tooth, dental bridges are more functional and more suitable when several missing teeth need to be replaced. A dental bridge is made up of one or more dental crowns on either side of the prosthetic teeth. The anchoring teeth over which the dental crowns are to be placed are called “abutment” teeth and the prosthetic teeth placed where the missing teeth used to be are called the “pontics”.

Who Is The Right Candidate To Get Dental Crowns And Dental Bridges?

What To Expect At The Consultation?

Depending upon your case, your first appointment for dental crowns or dental bridges will be scheduled. A dental crown or dental bridge procedure generally requires two appointments.

Procedure To Get Dental Crowns And Dental Bridges

At Fisher & Orfaly Dental, in Salem, MA, your first appointment for dental crowns and dental bridges will include taking several highly accurate molds (or impressions) that will be used to create your custom dental crown. A mold will also be used to create a temporary dental crown or temporary dental bridge which will stay on your tooth for approximately two weeks until your new dental crown is fabricated by a dental laboratory.

During the first step of the dental crown procedure, the dentist will prepare the tooth by removing any decay and shaping the surface to properly fit the crown. Once these details are accomplished, your temporary dental crown will be placed with temporary dental cement (also called dental glue) and your bite will be checked to ensure you are biting properly. Your temporary crown will be removed at your second appointment, the tooth will be cleaned, and your new dental crown will be carefully placed with permanent dental glue to ensure the spacing and bite are accurate.

While various dental crowns types are available, ceramic crowns, also called porcelain crowns, are the most popular. They are usually recommended when the dental crown needs to be placed over one of the front teeth. Ceramic crowns or porcelain crowns are highly durable and will last many years, after which they can be replaced. Ceramic crowns or porcelain crowns are made to suit the structure, shape, and size, and color of your natural teeth. Another popular choice is the porcelain fused to metal crown, which is a dental crown with a metal-alloy interior and porcelain exterior. Porcelain fused to metal crowns have the strength of metal crowns and the aesthetic of porcelain or ceramic crowns, and they are more affordable than porcelain or ceramic crowns.

Dental bridges consist of two crowns - one on either end - that are fused to existing healthy teeth or attached to dental implants. The existing adjacent teeth that act as anchors for the dental teeth are called “abutment teeth.” A set of replacement teeth called “pontics” rest in between the crowns and are used to cover the gap caused by the missing teeth. The first step in the dental bridge procedure is the preparation of the abutment teeth. The dentist will remove enamel and dentin from the abutment teeth to make space for the dental crowns. Then a temporary bridge will be placed over those teeth with the help of temporary dental glue. The temporary dental bridge and crowns will protect the abutment teeth until the permanent dental bridge and crowns are ready. During the second appointment for a dental bridge, the dentist will remove the temporary crowns and bridge, and carefully place the permanent dental bridge with permanent dental glue. The most popular types of dental bridges are traditional dental bridges and cantilever dental bridges. Implant-supported dental bridges are considered to be the most stable, but they are also the most expensive. Also, the procedure to get implant-supported dental bridges is very lengthy. Two surgeries are required during the treatment (one to place the implants, second to place the bridge) and so the treatment will take many months to be complete.

How Is The Recovery After Getting The Dental Crowns And Dental Bridges?

Dental crown recovery and dental bridge recovery take a few days as the body recovers from the inflammation from the procedure. You may have soreness or sensitivity in the gums and your mouth may feel tender immediately after the treatment. This may be because of the dental glue used or the reshaping of the affected tooth during the procedure. Fortunately, the soreness will most likely go away after a few days.

You are allowed to resume your day-to-day activities usually a day after the appointment. The soreness of gums and bleeding in a few cases may take a few weeks to heal.
